Who and What is Home Hospice
Think of Hospice as a resource - a varied team of healthcare professionals who work with you to develop and provide a comprehensive, individualized plan of care. We'll help involve you and your family in the direct care of your loved one - and we'll be there to offer support and respite when needed.
Our hospice team is available on call 24 hours a day, 7 days a week. With 24-hour pharmacy access, we coordinate the orivision if pain medication, symptom control, and help you access equipment and supplies, including beds, wheelchairs and other medical equipment.
Home Hospice Admission
VNA Home Health Care accepts referrals from physicians, hospital discharge planners and social workers as well as directly from patients and families.
