What is Mental Health Homecare?
Our mental health program treats adults who have a history of acute or chronic psychiatric illnesses, substance abuse, or co-occurring disorders. Our team is trained to provide a holistic approach, focusing on healing rather than curing. We strive to help individuals achieve and maintain the highest form of wellness while working toward as much independence as possible. Ongoing education and support are also available to meet the needs of family members.

People with a psychiatric diagnosis who are unable to access outpatient treatment in a safe and effective manner may qualify for skilled mental health services in their home. Our mental health program offers a range of therapeutic interventions. We work closely with hospitals and community organizations to ensure continuity of emotional, psychological and medical care.

Our Mental Health Team includes:
  • Certified psychiatric nurses
  • Licensed clinical social workers
  • Home health aides
  • Occupational therapists
  • Nutritionists

Admission to the Mental Health Program
VNA Home Health Care accepts referrals from physicians, hospital discharge planners, social workers, community mental health programs, and substance abuse programs as well as from individuals or family members.

The following list includes reasons why someone might be referred to our program:

  • Living at home and unable to access mental health services
  • Repeated psychiatric hospitalizations
  • Follow-up after a hospitalization
  • Monitoring medications
  • Safety assessments

Services for individuals who qualify for mental health home care are covered by Medicare, Medicaid, and most commercial insurers. We work closely with the client's physicians, other community mental health providers, and family members to assess individual needs and eligibility, and to develop a coordinated plan of care.

  • Crisis intervention to avoid or reduce hospitalizations
