VNA Home Health & Hospice provides the Medicare certified home care needed to support your medical needs in your own home. Effective January, 2011, VNA will also provide companionship care, independent living services and geriatric care management through LifeStages, a new division of VNA.

Our broad array of home care services can be brought together to provide a unique plan of care designed to focus on wellness, independence and returning you or your loved one to health. For our hospice clients, we focus on quality of life providing support and comfort to our client and their family at the end of life.

Home Health services

Home Health services are ideal for someone who has just returned from a stay in the hospital or rehabilitation center or anyone who has developed a medical issue that can be treated at home. Our team of nurses, home health aides, therapists and social workers work with you in your own home to help you return to wellness as soon as you are able.

Hospice services

Hospice services provide the caring attention needed for individuals and their families facing terminal illness. When the decision has been made to receive hospice care, our Hospice team works to support the patient’s quality of life, minimize pain and maximize comfort -- all while maintaining them safely in the comfort of home. We also focus on the family providing support and comfort through this difficult time.

Telehealth services

Telehealth services provide important support for patients with chronic illnesses who need daily oversight to remain healthy at home. Our telehealth service provides the client and their family peace of mind knowing that, even when there isn’t a medical expert in your home, there is one overseeing your care on a daily basis. Our nurses review your vital statistics daily and communicate with you regularly. They will know when everything is looking normal or when your physician needs to be called. Our telehealth service allows you to remain safely at home knowing that you are not alone.

LifeStages services

LifeStages services are designed to help elders maintain their independence in their own homes. We offer companionship care, meal preparation, light housekeeping, errands, personal care and many more services that provide that little bit of support that can make all the difference for elders living at home. Our services are also designed to assist families as they struggle with the challenges of long term care. By communicating with families regularly, our geriatric care managers are there to guide and support families, either locally or from a distance, partnering with them to ensure their loved one is receiving the highest quality care. In addition, we offer Lifeline as increased security for those elders living at home.
