Home Health Care Services

Our Services of Programs
From monitoring the complications of diabetes, to sharing home safety tips, to helping families cope with a terminal illness, VNA Home Health Care has an interdisciplinary team of professionals who bring compassion and clinically excellent care into the home. Working in collaboration with your primary care physician, we will develop and provide you with a comprehensive plan of care developed specifically to fit your needs.

It begins with an initial assessment, which you or a family member can request. We will meet with you to discuss your situation, decide the level of care you need and determine what outside resources - from your family to agencies in the community - are available to help you.

Our Home Health Care Team Includes:
  • Skilled Nurses
A full range of nursing services including wound care, IV therapy, diabetic care, catheter care, cardiac and respiratory assessment, medication management, pain management and cancer care.
  • Therapists
Assessment and treatment by physical therapists, occupational therapists and speech language pathologists for adults and children.
  • Medical Social Workers
Case management, grief counseling, coordination of community resources, and assistance with social / emotional effects of acute and chronic illness.
